TOMF Awards $1,000 to Green Valley Assistance Services, Inc.

The Tucson Osteopathic Medical Foundation is proud to announce it has awarded its March Trustee Award in the amount of $1,000 to Green Valley Assistance Services, Inc. in collaboration with United Community Health Center and A.T. Still University.  The Award will provide funding for their prevention assessment tool and healthy lifestyle educational materials. The overall goal of this initiative is to encourage seniors to become active participants in their health and to improve communication between seniors and their primary care physicians.

Elderly patients use health services more often than the general population and typically have more complex needs. Due to this, there is a greater risk of receiving fragmented services, which can lead to a potential crisis. To aid in averting crises, this community based program aims to decrease healthcare expenses and emergency room admissions and re-admissions for elderly patients by ensuring follow-up with patients after discharge.  Through the initiative, seniors will have an increased chance to obtain a healthy lifestyle though prevention models in the home, improved access to care, increased communication with their primary care team and the utilization of RN Care Advocates. The RN Care Advocates are professionals who will create customized lifestyle plans for them based off the data collected and analyzed by the initiative.
